North Sedgemoor PCN is seeking a Care Coordinator to work within its Primary Care Network and be part of the wider social prescribing team. You will help coordinate care, manage referrals, and support MDT meetings across several practices in the network.

The role focuses on promoting personalised care, improving continuity, and working closely with GPs, nurses, and other professionals. A flexible, multi-disciplinary environment with opportunities for training is offered.

Competenze

• Minimum two years Primary Care experience in administration or reception roles.
• Experience with EMIS Web and other clinical systems such as AccuRX and C the Signs.
• IT literate, including ability to use Microsoft Office (Outlook, Word, Excel, Teams, PowerPoint).
• Understanding of handling confidential information and patient data.
• Able to work as part of a multi-disciplinary team.

Mansioni

• Arrange the daily PCN-led MDT meetings and coordinate integrated care.
• Take notes of MDT meetings, disseminate actions, and chase progress.
• Manage reporting requirements within NHSEDES specifications.
• Receive and collate information from transfers of care and present to the MDT.
• Signpost service users to relevant services and support referrals as needed.
• Support with administration and patient management across the PCN.
