Health Village & Dearne Primary Care Network (PCN) are seeking to recruit an Advanced Clinical Practitioner with a Mental Health Qualification to enhance our current team within the PCN.

Health Village & Dearne PCN is a partnership of three practices: St Ann's Medical Centre, Market Surgery, and Clifton Medical Centre. We serve a patient population of 48,000 with a diverse team of staff.

Main duties of the job

The post holder is an experienced clinician with a mental health qualification, working within their professional boundaries to provide patient care collaboratively with the team. Responsibilities include initial history taking, clinical assessment, differential diagnosis, interpreting investigations, and planning treatments, which may involve self-care or referrals to specialist teams.

Job responsibilities

The core responsibilities of the Advanced Clinical Practitioner include:

1. Practise in accordance with professional conduct codes and within scope of practice, being responsible and accountable for decisions and actions.
2. Demonstrate understanding of responsibility, autonomy, and limits of competence, especially with complex or uncertain situations.
3. Make professional judgments about when to seek help, reflect critically on practice, and maintain self-awareness and emotional intelligence.
4. Work with individuals, families, and carers using appropriate assessment methods.
5. Communicate effectively to support decision-making and care planning.
6. Use expertise to inform clinical reasoning and decision-making.
7. Initiate, evaluate, and modify interventions, including prescribing medicines, therapies, and lifestyle advice.
8. Manage risk professionally and exercise judgment accordingly.
9. Handle patients with acute and chronic conditions, providing expert advice.
10. Develop and evaluate treatment plans for chronic diseases.
11. Identify and support patients at risk of long-term conditions, preventing adverse health effects.

Qualifications
