Job description

Primary Care Mental Health Practitioners (Mental Health Additional Roles Reimbursement Scheme) - Bradford PCN4 Primary Care Network (PCN)

This role offers an exciting opportunity for a registered band 6 mental health professional (Nurse, Social Worker, or OT) to work dynamically across primary and secondary care. The aim is to integrate mental health pathways and enhance understanding of mental health within primary care.

The role involves providing holistic assessments, linking individuals to community and VCSE resources, and facilitating seamless transitions into secondary care when necessary. The practitioner will also participate in multidisciplinary meetings held by GP practices within primary care.

The primary care mental health service is expanding as part of our community mental health transformation. The role will closely align with the community transformation initiatives and the core team established for this purpose. The successful candidate will be based at GP sites within the PCN during working hours.

Role Objectives
  • Promote understanding and knowledge of mental health and wellbeing within primary care to enable the multidisciplinary team to provide integrated, holistic support.
  • Lead on specialist assessments of service users with mental health issues within primary care.
  • Provide signposting based on holistic assessments, utilizing a range of resources including VCSE and community organizations.
  • Deliver evidence-based, time-limited interventions within scope of practice and experience.
  • Facilitate swift and seamless transitions into other mental health services as needed.
  • Collaborate with secondary mental health services to support effective transitions from secondary to primary care.
  • Work with service users and families to support shared decision-making, recovery, and self-care.
  • Contribute to the development of innovative support models within primary and secondary mental health services.
