A forward-thinking organization is seeking a dedicated Primary Care Mental Health Practitioner to enhance mental health pathways within primary care. This role offers an exciting opportunity for a registered band 6 mental health professional (Nurse, Social Worker, or OT) to work dynamically across primary and secondary care. The aim is to integrate mental health pathways and enhance understanding of mental health within primary care.
The role involves providing holistic assessments, linking individuals to community and VCSE resources, and facilitating seamless transitions into secondary care when necessary. The practitioner will also participate in multidisciplinary meetings held by GP practices within primary care.
The primary care mental health service is expanding as part of our community mental health transformation. The role will closely align with the community transformation initiatives and the core team established for this purpose. The successful candidate will be based at GP sites within the PCN during working hours.
