We’re working with a large, well-established GP practice in Leeds that’s looking for a passionate and skilled Lead Practice Nurse to join their friendly, multi-disciplinary team.

What You'll Be Doing:

• Lead and support a diverse nursing team
• Deliver high-quality care in areas such as diabetes, COPD, cardiovascular health, wound care, immunisations, and women’s health
• Champion chronic disease management and health promotion initiatives
• Support service development and continuous quality improvement
• Contribute to the leadership and mentoring of junior staff

What We’re Looking For:

• Experienced Practice Nurse with leadership capabilities
• Ideally trained in long term condition management
• A Prescribing Qualification is advantageous (or willingness to work towards it)
• Strong communicator, team player, and proactive clinician
• Passionate about patient care and clinical excellence
