Job summary

Miles Platting, Newton Heath & Moston PCN would like to recruit a full time Advanced Clinical Practitioner to complement our current clinical workforce.

We are looking for an experienced and motivated Clinician to join our existing team to support our GPs and the wider primary care workforce. Working within your professional boundaries, you will be responsible for working collaboratively with the multi-disciplinary primary care teams the PCN.

Main duties of the job

Autonomous and independent working.

To ease practice workload by seeing a variety of patients within a clinical setting.

Conduct Structured Medication Reviews

Identification of residents of concern for preventative/proactive admission avoidance

Manage a caseload of complex clients using advanced problem-solving skills and advanced clinical reasoning skills

To maintain accurate documentation and patient records

To work alongside and support the MDT

Attend and engage in educational sessions

Assess and holistically clinically examine patients with both minor and complex needs through history taking, physical examination and appropriate use of investigations to confirm diagnosis.

Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.

Refer and discharge safely ensuring that safety-netting advice is offered.

Triage telephone referrals, calls from external clinicians, patients and others into a range of services appropriately as required.

Plan, implement and evaluate an effective management plan for the patient, ensuring safe discharge/referral and/or transfer to an appropriate health care service.
