Job summary

Rotherham Doncaster and South Humber NHS Foundation Trust is seeking a Positive Behaviour Support Practitioner to join the Community Learning Disability Team in Doncaster. Qualifications

• Complete specialist functional assessments and formulations for people with learningdisabilities and autism who present with behaviours of concern.
• Develop, implement and review Positive Behaviour Support plans in partnership withindividuals, families, carers and professionals.
• Provide specialist PBS advice, guidance and consultation to colleagues, supportproviders and partner agencies.
• Manage a defined caseload and contribute to holistic assessment, intervention andcare planning within the MDT.
• Deliver PBS-related training, coaching and support to staff, carers and otherstakeholders.
• Promote trauma-informed, evidence-based and person-centred approaches in allaspects of practice.
• Contribute to service development, clinical governance, quality improvement and auditactivities.
• Maintain accurate clinical records and ensure compliance with safeguarding,confidentiality and professional standards.
