Job summary

HealthHarmonie is seeking a Patient Pathways Administrator to support admissions, referrals, and patient care through the service.

You will process notes after triage, update systems, handle GP/hospital queries, track outcomes, and liaise with laboratories. The role emphasises organisation, communication, and teamwork.

The position offers a pension, 25 days leave, paid overtime, and ongoing training within a supportive NHS-aligned environment.

Qualifications

• Strong organisational and administrative capabilities.
• Excellent written and verbal communication skills.
• Experience working independently and as part of a team.

Responsibilities

• Process patient notes after triage to determine admission or onward referral.
• Utilise the National EReferral Service.
• Update patient administration systems.
• Answer GP/hospital/laboratory queries by phone and email.
• Review patient outcome reports and take action as required.
• Chase laboratories for outstanding histology results.
• Manage departmental actions in line with KPI’s.
• Update patient care trackers using Excel.
• Confirm patient appointments by telephone.
