Croydon Health Services NHS Trust is seeking a Band 4 Patient Pathway Coordinator for a 12-month fixed-term role within the Dermatology and Plastics POD 4 team. An exciting opportunity has arisen for a Band 4 Patient Pathway Coordinator to join the Dermatology and Plastics - POD 4 team.
The post holder will work as part of a multidisciplinary Point of Delivery (POD) team, providing proactive support in managing patient pathways from referral through to discharge. They will ensure pathways are managed in a timely and efficient manner and act as a key point of contact for patients regarding all administrative aspects of their care.
The post holder will act as the main point of contact for all administrative matters relating to patients' pathways of care. They will work as part of a multidisciplinary team, providing expertise in the proactive management of patient pathways from referral through to discharge, ensuring processes run smoothly and efficiently.
The post holder will also be responsible for supervising and allocating work across the POD (Point of Delivery) team. They will promote high levels of patient and clinician satisfaction by being an accessible, customer-focused, and knowledgeable point of contact.
In line with Trust standards, the post holder will follow Standard Operating Procedures (SOPs) to ensure all aspects of the role are carried out accurately and within agreed timescales.
