Job summary

Mid and South Essex NHS Foundation Trust is recruiting a Patient Pathway Co-Ordinator Specialist Medicine at Band 4 for a 12-month fixed-term contract. The role is based at the Chelmsford site, offering 37.5 hours per week with a salary of £28,392 per annum (pro rata for part-time).

You will support outpatient pathways, coordinate clinics, and drive improvements in patient flow. You will work with clinical and admin teams to streamline pathways, reduce waiting times and enhance patient care

Qualifications

• Significant administrative experience in a healthcare setting.
• Up-to-date knowledge of key access targets e.g. 18 weeks RTT, 2 Week Wait.
• Experience with hospital IT systems and MS Office products.

Responsibilities

• Manage PTL lists by specialty, liaising with consultants, nurses and admin team.
• Ensure clinic schedules are up to date and shared promptly.
• Handle patient enquiries, forwarding queries per Trust policy and equality/confidentiality standards.
• Produce and maintain tracking spreadsheets and performance reports.
• Proficient in Trust systems and MS Office tools to support daily duties.
• Plan daily tasks, prioritise work to meet KPIs with some autonomy.

Are you passionate about digital transformation and improving patient care? We’re looking for a motivated and detail-oriented Patient Pathway Co-Ordinator to support and improve the pathways within Specialist Medicine

This is a 12-month fixed-term opportunity to play a key role in transforming how we manage outpatient demand, helping to streamline pathways and reduce waiting times for patients. You'll work closely with operational, administrative, and clinical teams to ensure we continue to take positive steps in making a difference to our patients.

Main duties of the job

The management of PTL lists by specialty, liaising with consultants, nurses and administration team. Ensure effective maintenance of patient documentation and note holding systems.

Ensuring clinics schedules are up to date and sent across in timely manor

Deal with patient enquiries, either in person or by telephone forwarding queries to the appropriate person when necessary, taking into account Trust policy, with regards to equality and diversity, confidentiality etc.

Produce and maintain tracking spreadsheets and performance reports, escalating concerns where necessary.

Confident in the use of trust systems and MS software (Word, Excel, Powerpoint)

Plan daily duties and prioritse workload so this is completed in a timely manner and prioritised accordingly in keeping with KPI targets for the role. The postholder will work with some degree of autonomy and will be expected to problem solve for their patients within their remit.

Qualifications

• NVQ 4 or equivalent experience
• A Level in English and Maths

• Experience of working with IT office packages
