Job summary

Nottinghamshire County Council is seeking a motivated individual to join the countywide Continuing Healthcare (CHC) team. The post focuses on MDT attendance to carry out Decision Support Tools and ensure personalised care and proportionate support for residents.

The role requires CHC knowledge, the National Framework and Care Act section 22 experience, strong written communication, and the ability to work in a dynamic, collaborative environment across Health, Finance and Service Improvement.

Qualifications

• Working knowledge of CHC, the National Framework and Section 22 of the Care Act 2014.
• Ability to thrive in a fast-paced, dynamic environment.
• Strong written recording skills and confidence in professional conversations.

Responsibilities

• Attend Multi-disciplinary Teams (MDTs) to undertake Decision Support Tools (DSTs).
• Collaborate with colleagues in Health, Finance and Service Improvement to improve outcomes for residents.
• Maintain high-quality written records and communicate clearly with partners.
