Job summary

ICO Health Group is recruiting a GP Care Navigator for a 12-month fixed-term contract to join a busy practice team in Downham. The role focuses on front-line patient contact, ensuring patients are directed to the most appropriate healthcare professional or service, and supporting clinical teams to maintain an efficient service.

As the first point of contact, you will signpost to GPs, nurses, pharmacists, social care and voluntary organisations, while promoting self-care and patient independence.

Qualifications

• Experience in primary care or clinical front-line setting is essential.
• Strong interpersonal and customer service skills are required.
• Understanding of confidentiality and GDPR is desirable.

Responsibilities

• Act as the first point of contact for patients via face-to-face, telephone, and digital platforms.
• Signpost patients to appropriate clinicians or services, including GPs, nurses, pharmacists, social care, and voluntary organisations.
• Support patients in accessing NHS and community services.
• Maintain accurate patient records and support data entry and reporting.
• Book, amend, and manage appointments and home visit requests.
• Promote self-care and patient independence.
