Job description

We require a motivated and flexible administrator to support the day to day running of team. Tasks include answering and directing phone calls to the most suitable team member, taking basic details of referrals and advising on expected time frames for response. Supporting with our core data collection and analysis, and supporting with stock management and other tasks to support our clinicians to provide a high quality service.

The Pathfinder Team is a specialist community team based within North Devon District Hospital, consisting of professionals from therapy, nursing and paramedic backgrounds. Our remit is to holistically assess and then procure onward care, give advice, issue equipment, support with therapy needs etc. for specific patient groups, these include:

  • Short stay admissions ward discharges
  • Frailty Acute Hospital at Home
  • End of life care discharges

Our role can be challenging and is time sensitive in order to support patient flow through the acute hospital; but it is also rewarding and varied.

We work closely with our community colleagues and have established ourselves as a link between acute and community care in order to provide seamless transition of care between hospital and home or other receiving destination.

We require a forward thinking, dynamic, motivated and creative individual to join us. Being able to work as part of a team and under pressure are essential skills that the successful candidate will hold. Experience of the trusts computer systems and good working knowledge of standard applications and programmes will be beneficial. Training and support will be given to the successful applicant. We are a friendly, flexible and supportive team, who face challenges together and ensure we support each other through good times and difficult times.
