The Pathfinder Team is a specialist community team based within North Devon District Hospital, comprising professionals from therapy, nursing, and paramedic backgrounds. Our remit includes:
Our role is challenging and time-sensitive, supporting patient flow through the hospital. It is rewarding and varied.
We work closely with community colleagues, acting as a link between acute and community care for seamless patient transitions.
We seek a forward-thinking, dynamic, motivated, and creative individual. Ability to work as part of a team and under pressure is essential. Experience with trust computer systems and standard applications is beneficial. Training will be provided. We are a friendly, flexible, and supportive team.
