Advanced Clinical Practitioner role at Ashton GP Service within gtd healthcare. 30 hours per week, Wednesday and Friday, 08:00–18:30. Salary £57,625–£71,281 per annum pro rata, with annual increments and NHS Pension provider.

A supportive, community-focused practice with strong clinical leadership and an experienced MDT team. You will join a teaching environment, supporting trainee ACPs and learners, with opportunities for specialist interests and ongoing development within our not-for-profit

Qualifikationen

• ACP with MSc Advanced Clinical Practice.
• Commitment to delivering high quality care.

Aufgaben

• Assess and holistically examine patients with minor and complex needs; perform history taking, physical examination and investigations.
• Plan, implement and evaluate management plan; safe discharge/referral/transfer.
• Provide information on prescribed or OTC medications; discuss regimens, side-effects and interactions.
• Prioritise health problems and intervene in urgent or emergency situations; initiate emergency care when needed.
• Refer and discharge safely with safety-netting advice.
• Triage telephone referrals and external requests to appropriate services.
