Job summary

gtd healthcare is hiring an Advanced Clinical Practitioner for Gordon Street Medical Centre in Ashton-in-Makerfield. The role is 15 hours per week, Wednesday and Friday, 08:00–18:30, with strong clinical leadership and a collaborative MDT.

You will manage acute and routine presentations, perform assessments, prescribe, and contribute to service development within a supportive team. Benefits include on-site parking, wellbeing support, and cycle-to-work schemes.

Qualifications

• Experience of working as senior clinician.
• Accountability of own role and other roles in a practitioner-led service; experience in leading and managing quality improvement.
• Experience in leading/managing a team of nurses.
• Experience of working in an unscheduled care service.
• Experience of having worked in Primary Care.

Responsibilities

• Assess and holistically clinically examine patients with both minor and complex needs through history taking, physical examination and appropriate use of investigations to confirm diagnosis.
• Plan, implement and evaluate an effective management plan for the patient, ensuring safe discharge/referral and/or transfer to an appropriate health care service.
• Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.
• Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
• Refer and discharge safely ensuring that safety-netting advice is offered.
• Triage telephone referrals, calls from external clinicians, patients and others into a range of services appropriately as required.
