Job summary

Changing Lives in Durham is seeking a Part Time Project Worker to provide person-centred support to males 18+ with complex needs, including mental health challenges and substance misuse. The role focuses on key working, developing and reviewing support plans, and promoting recovery.

You will build relationships with clients, coordinate activities, maintain records, and ensure a safe, welcoming environment through regular project housekeeping.

Qualifications

• Providing person-centred support to males aged 18 and over with a range of complex needs, including mental health challenges and substance misuse issues.
• Role includes key working, developing and reviewing support plans, completing risk assessments, and promoting independence, wellbeing, and recovery.
• Coordinating meaningful activities to encourage engagement and social inclusion, maintaining accurate records, and ensuring a safe, clean, and welcoming environment through regular project housekeeping duties.
• Building positive relationships, working collaboratively with multidisciplinary teams, and supporting individuals in achieving their personal goals and positive outcomes
