Job summary

• Develop and review support plans, complete risk assessments, and promote independence, wellbeing and recovery.
• Coordinate meaningful activities to encourage engagement and social inclusion, maintain accurate records, and ensure a safe, welcoming environment through regular housekeeping duties.
• Build positive relationships and work with multidisciplinary teams to support individuals in achieving personal goals.
