Handforth Health Centre is seeking an enthusiastic Practice Nurse to join a small, friendly team of two nurses and three HCAs. The role is part-time (up to 25 hours per week) to cover maternity leave until March 2027. Salary negotiable depending on experience.

Prior primary care experience is essential and you may cover an extended Wednesday evening shift with another nurse and GP. The successful candidate will deliver nursing care across chronic disease management, health promotion,

Competenze

• Registered Nurse with NHS primary care experience and essential chronic disease management
• Experience working as a practice nurse or community nurse is essential
• Strong knowledge of evidence-based practice and clinical governance

Mansioni

• Deliver chronic disease clinics (CHD, etc.) and immunisations.
• Carry out health promotion and preventive nursing interventions.
• Manage travel medicine services and routine nursing care per evidence-based guidance.
• Maintain accurate clinical records and chronic disease registers.
• Support safeguarding and liaise with external agencies as needed.
• Work as part of a multidisciplinary team and cover extended hours as required.
