Job summary

Northern Healthcare Limited in Rochdale is seeking a part-time Mental Health Support Worker to join Moss Lodge, providing practical, emotional and person-centred support to adults with mental health needs, autism and learning disabilities.

You will help individuals access the community, attend appointments and participate in meaningful activities, while building relationships and supporting independence in a welcoming setting.

Responsibilities

• Supporting appointments: Accompanying individuals to GP appointments, hospital visits, health reviews and other important commitments.
• Planning activities: Helping organise and deliver activities within Moss Lodge that encourage social interaction, confidence, creativity and wellbeing.
• Getting out and about: Supporting people to explore the local Community, enjoy walks, visit local places, attend activities or simply enjoy a change of scenery.
• Building relationships: Spending quality time with the people we support, getting to know their personalities, interests and aspirations.
• Promoting independence: Encouraging individuals to develop everyday living skills, make their own choices and build confidence.
• Providing emotional support: Being someone who listens, understands and offers reassurance when needed.
• Recording and communicating: Maintaining accurate records and sharing important information with colleagues to ensure consistent support.
• Promoting safety and wellbeing: Following individual support plans, risk assessments, safeguarding procedures and company policies.
