Job summary

NHS Grampian invites applications for two part-time Assistant Practitioner posts in the Community Nursing Team, covering Kincardine and Mearns District Nursing Teams. You will work under Registered Nurses to deliver patient-centred care in homes and community settings.

We seek candidates with SCQF Level 7 (or equivalent) and PEMAP training (or willingness to complete). A permanent role with development opportunities, supportive teamwork, and a relocation allowance up to £8,000 is offered.

Qualifications

• Holds a relevant qualification (SCQF Level 7 or equivalent).
• Has completed PEMAP training or willingness to complete this.
• Experience working within a healthcare setting.
• Excellent communication and interpersonal skills.
• Ability to work independently and in a multidisciplinary team.
• Compassionate, caring and professional approach.
• Good organisational and time management skills.

Responsibilities

• Deliver delegated clinical interventions and treatments.
• Support patients with long-term conditions and rehabilitation goals.
• Undertake clinical observations and monitor patient progress.
• Assist with wound care and other delegated nursing procedures after competency assessment.
• Promote health, wellbeing and independence.
• Maintain accurate patient records and documentation.
• Collaborate with colleagues, patients, carers and partner agencies.
