An exciting opportunity has arisen for motivated and enthusiastic Assistant Practitioners to join our Community Nursing Team. Working as part of a multidisciplinary team, you will play a vital role in supporting the delivery of high-quality, person-centred care to patients within their own homes and community settings.

This is an excellent opportunity for an experienced Healthcare Support Worker or Assistant Practitioner who is passionate about delivering compassionate care and improving patient outcomes.

The Role

As an Assistant Practitioner, you will work under the direction and supervision of Registered Nurses whilst exercising a degree of autonomy within your scope of practice. You will contribute to the assessment, planning, delivery and evaluation of care, supporting patients with a wide range of health needs.

Your Responsibilities Will Include

• Delivering delegated clinical interventions and treatments.
