Job description

We invite high calibre applicants to embrace this exciting opportunity to join the dynamic and enthusiastic multidisciplinary team within the Aberdeen City Hospital at Home Service as we continue with our planned service expansion.

We are recruiting to both permanent and fixed-term (12 month) secondment opportunities within our Advanced Practice workforce and welcome applications from experienced Advanced Nurse Practitioners (ANPs) and Advanced Clinical Practitioners (ACPs) looking to contribute to the ongoing development of this innovative service.

Hospital at Home provides short duration episodes of high-quality, hospital-level acute care to patients within their own homes, avoiding hospital admission where appropriate, reducing the duration of inpatient stays, and supporting the wider health and care system to improve capacity, resilience and patient outcomes.

The service is currently based at Woodend Hospital and provides clinical cover seven days a week between 0800 and 2015 hours across Aberdeen City. As the service continues to develop, we have also begun providing care to selected areas of Aberdeenshire.

This is an exciting opportunity for an experienced Advanced Practitioner who is passionate about delivering high-quality acute care in community settings. Excellent professional leadership, effective multidisciplinary team working across professional boundaries, and evidence of significant consolidation and application of advanced clinical assessment, diagnostic reasoning, clinical decision-making and non-medical prescribing skills are essential to this role.

Working autonomously and as part of a multidisciplinary team, the successful candidate(s) will be responsible for the assessment, diagnosis, treatment and ongoing management of patients with acute and complex healthcare needs within their own homes. You will support the delivery of safe, effective and person-centered care whilst contributing to service development and innovation within a rapidly expanding model of care.

The post holder will be expected to demonstrate experience in, and be keen to continue supporting, the professional development of others through teaching, mentorship and clinical supervision. Evidence of their own continuing professional development (CPD) and engagement in research, audit/ quality improvement activity is essential, underpinned by a strong understanding of current healthcare priorities and issues affecting Advanced Practice. Successful candidate(s) will also be required to be flexible to the needs of the role and service, working closely with the Leadership Team to support the continued development of the Hospital at Home service.

Posts are available on both a permanent basis and as fixed-term secondment opportunities. The ability and willingness to travel throughout Aberdeen City and Aberdeenshire areas covered by the Hospital at Home service in varying weather conditions is essential. Applicants must therefore hold a full UK driving licence and have access to a vehicle for work purposes, and should be aware that the service operates within Aberdeen City Low Emission Zone.
