Job summary

NHS Grampian Fraserburgh invites applications for an experienced District Nurse (Band 7) to join our Community Nursing Service. You will manage a complex caseload and deliver high-quality person-centred care to help people remain independent at home.

The post requires current NMC registration, a Specialist Practitioner Qualification in District Nursing, and substantial community nursing experience. A full UK driving licence and access to a vehicle for work are essential; interviews are planned

Qualifications

• Be a Registered Nurse with current NMC registration.
• Hold a Specialist Practitioner Qualification in District Nursing.
• Have significant experience working within community nursing services.
• Demonstrate excellent clinical assessment and decision-making skills.
• Have experience managing patients with complex and unpredictable healthcare needs.
• Possess strong communication and interpersonal skills.
• Be committed to delivering person-centred care and continuous service improvement.

Responsibilities

• Deliver advanced clinical assessment, planning and evaluation of care for patients with complex healthcare needs.
• Manage and coordinate care to support individuals to remain safely at home where appropriate.
• Provide specialist district nursing expertise to patients, families and carers.
• Undertake complex wound management, palliative care, long-term condition management and other community nursing interventions.
• Work in partnership with GPs, Allied Health Professionals, Social Care and wider multidisciplinary teams.
• Contribute to service development, quality improvement initiatives and evidence-based practice.
• Support and supervise junior staff, students and healthcare support workers as required.
• Maintain high standards of clinical governance, patient safety and professional practice.
