Job summary

Leaders in Care is seeking a Clinical Pharmacist to support medicines optimisation across the patient journey in a busy hospital setting. This role is part-time (18 hours per week) with a pro rata salary up to £50,000.

Qualifications

• Registered Pharmacist with the GPhC.
• Hospital pharmacy experience and a clinical diploma or equivalent experience.
• Independent prescriber or experience in secondary care pharmacy.

Responsibilities

• Support medicines optimisation through medication review, reconciliation and prescription review.
• Provide expert medicines advice to patients, carers, consultants and staff.
• Assist with safe procurement, management and control of medication stock.
• Contribute to clinical governance, audits, incident reporting and introduction of new medicines and therapies.
