Job summary

Leaders In Care Ltd is seeking a Clinical Pharmacist for a hospital site, working 18 hours per week on a pro rata basis to enhance medicines optimisation across the patient journey.

The role requires hospital pharmacy experience, GPhC registration and a clinical diploma or equivalent qualification; an independent prescriber or secondary care experience is advantageous.

• Registered Pharmacist with the GPhC.
• Hospital pharmacy experience and a clinical diploma or equivalent.
• Independent prescriber or experience in secondary care pharmacy.

Responsibilities

• Support medicines optimisation through medication review, reconciliation and prescription review.
• Provide expert medicines advice to patients, carers, consultants and staff.
• Assist with safe procurement, management and control of medication stock.
• Contribute to clinical governance, audits, incidents and adoption of new medicines and therapies.
