Job summary

The Garden Surgery in Ledston is seeking a Care Navigator to join our friendly GP practice on a part-time basis. You will work 15 hours per week, with shifts typically starting mid-afternoon and finishing around 18:00–18:30, across Monday to Friday.

You will support patients to access care, manage appointments and clinical information, and perform a mix of patient-facing and administrative tasks using systems like SystmOne and NHS Mail. Training and a collaborative team environment are provided.

Qualifications

• Experience in a customer service, patient or public facing role
• Experience in a busy or fast-paced environment
• Experience using IT systems in a work setting
• Desirable GP Practice, primary care, or NHS experience
• Experience with online services or digital access

Responsibilities

• Provide a first point of contact for patients by telephone, in person and online
• Support patients in accessing appropriate services and signpost as needed
• Book, amend, and cancel appointments using the clinical system
• Maintain accurate patient records and handle confidential information
• Use systems such as SystmOne, SystmConnect, PATCHS and NHS Mail
• Chaperone duties when required and escalate urgent matters
