Job summary

GP Practice is seeking a Care Navigator to support the clinical team by signposting patients to the right healthcare professional and handling reception duties. The role involves triage, patient registration, appointment management, and data entry within EMIS/DOCMAN systems.

The post is 25 hours per week across three days, with shifts on Wednesday, Thursday, and Friday, providing a vital link between patients and services in a busy community GP setting.

Responsibilities

• Signpost patients to appropriate healthcare professionals based on condition.
• Answer incoming calls and transfer or respond as needed.
• Process personal, telephone and e-requests for appointments.
• Handle incoming emails and correspondence as required.
• Accurately update patient records in the clinical system.
• Maintain and monitor the practice appointment system.
• Liaise with patients, team members and external agencies.
• Photocopy documents as required.
• Data entry for new and temporary registrations and patient information.
• Direct information requests (DSAR, solicitor letters, DVLA) to admin team.
• Handle queries efficiently and professionally.
• Undertake a range of administrative duties as needed.
• Be an integral part of the general practice team.
• Act as a chaperone when required.
