Hospice at Home West Cumbria is seeking a Healthcare Assistant to provide practical nursing care and emotional support in patients' homes and community settings in Workington and surrounding areas.

The role involves one-to-one care for individuals with palliative needs, under supervision, with duties spanning assessment, direct care, documentation and family support. Training and supervision are provided as part of ongoing professional development.

Competenze

• NVQ Level 2 or 3 in Health and Social Care
• Demonstrable clinical competence in palliative care or willingness to undertake additional training
• Awareness of Hospice at Home West Cumbria and care services
• Ability to communicate effectively with patients, families and health care professionals
• Ability to work in isolation without direct supervision for clinical tasks
• IT skills with Office applications and EMIs clinical systems
• Ability to travel across the geographical area covered by the post to attend meetings or training

• Understands that care complies with best practice and regulatory requirements to maintain quality and safety standards
• Performs initial Home Assessment and notifies team lead or on-call of risks
• Provides direct nursing care aligned to care plans for palliative/end-of-life patients
• Assesses patient condition during care episode and seeks advice for changes
• Assists with last offices as needed
• Provides emotional support to families and carers
• Participates in supervision and de-briefings as required
• Documents care in accordance with standards
• Works with hospice and induction of new staff and builds relationships
