TRINITY HOSPICE AND PALLIATIVE CARE SERVICES LTD is seeking a Healthcare Assistant to support patients and families in their own homes as part of the Community Nursing Team.

In this role you will provide practical, personal and emotional care in the last year of life, work with nurses and specialists, and help keep patients comfortable and well supported in their preferred place of care.

Responsabilités

• Deliver high-quality personal and practical care to patients in their own homes.
• Support admissions, discharges and ongoing care packages.
• Assist patients with washing, dressing, mobility and continence care.
• Monitor patients’ wellbeing and report any concerns or changes promptly.
• Provide comfort, reassurance and support to patients, families and carers.
• Contribute to care planning, handovers and multidisciplinary team meetings.
• Support patients and families through difficult conversations and bereavement.
• Maintain accurate clinical records and uphold confidentiality and dignity.
