Job summary

Northumbria Healthcare NHS Foundation Trust is recruiting a Nursing Assistant for the Palliative Care Support Team in Northumberland. The role delivers compassionate end‑of‑life care to patients at home, in the community and within hospital settings.

You will work with the MDT, liaising with patients and families while maintaining high standards of care and communication, including driving to patient visits when needed.

Qualifications

• NVQ Level 2 or 3 in care or equivalent qualification.
• Previous experience in health or social care setting.
• Valid driving licence required.

Responsibilities

• Provide care as planned by District Nurse or Macmillan Nurse for palliative care needs.
• Monitor patient’s condition and report changes to the Team Lead or District Nurse.
• Support families with emotional, physical and spiritual care.
• Record care in patients’ care plans in System One with accurate documentation.
• Work flexibly including days/evenings/weekends to deliver 365-day service.
