Job summary

Sage Global Services Limited is seeking a Triage Facilitator to lead the specialist clinical telephone triage for unplanned and virtual ward palliative care referrals across the St Giles Hospice catchment. The role operates seven days a week with initial hours 08:00–20:00 and potential 24/7 coverage in phase two.

The post holder will make complex clinical decisions, supervise staff, and ensure safe, timely care across pathways within professional scope.

Qualifications

• Adult nursing qualification and HCP registration.
• Non-medical prescriber status required.
• Degree-level education with progression toward Level 7.

Responsibilities

• Lead clinical telephone triage service for palliative care referrals.
• Provide triage leadership and supervise Band 6 colleagues.
• Coordinate urgent referrals and safe care pathways.
