Job summary

St Giles Hospice invites applications for a Clinical Nurse Specialist – Rapid Response Service based in Whittington/Sutton Coldfield. You will respond to urgent referrals, perform rapid palliative care assessments at patients' homes, and develop immediate care plans.

The service operates across Staffordshire St Giles catchment, with seven days a week hours from 08:00 to 20:00, expanding to 24/7 in phase two.

Qualifications

• Adult Registered Nurse or relevant HCP qualification.
• Health assessment qualification or working towards it.
• Palliative/end-of-life care qualifications.
• Clinical supervision experience.
• Clinical governance knowledge.
• Teaching skills.

Responsibilities

• Respond to urgent referrals with rapid specialist palliative care assessments.
• Develop immediate care plans for urgent needs.
• Support management of complex care pathways and audit.
• Provide staff supervision and coordinate service flow.
• Collaborate with GPs, District Nurses and other teams.
