Job summary

St Giles Hospice is seeking an experienced lead for the telephone triage service, overseeing rota management and clinical supervision across shifts. You will perform specialist palliative assessments by phone, prioritise referrals, and coordinate care plans with external teams.

The role requires an adult nurse or equivalent, a health assessment qualification, and a non-medical prescriber registration, with degree-level education and ongoing Level 7 study.

Qualifications

• Adult registered nurse or relevant HCP qualification.
• Health assessment qualification.
• Non-medical prescriber.
• Degree-level education, with evidence of level 7 study or working towards it.

Responsibilities

• Lead the triage team, oversee rota management and provide clinical supervision to Band 6 and wider staff.
• Carry out specialist palliative assessments by telephone and prioritise referrals by clinical need.
• Identify urgent or unmet needs and coordinate safe care and support plans.
• Triage referrals from 111/999, GPs, allied health professionals, community and hospital teams, patients and carers.
• Liaise with external services on shared care planning and escalation.
• Support personalised care planning, including ReSPECT and advanced care planning.
