Marie Curie is seeking two experienced Clinical Nurse Specialists to join a new service serving South Liverpool. You will manage a caseload of people with palliative-care needs, working across GP practices and community locations within the PCN to deliver coordinated, person-centred care.

You will lead on outreach, referrals, and build strong relationships with primary and community services, providing expert palliative-care guidance and ensuring patient wishes drive decision-making.

Qualifikationen

• Registered adult nurse with current NMC registration.
• Holds Nursing Degree and Level 6 post-registration qualification.
• Experience in specialist palliative care and complex case management.

Aufgaben

• Provide autonomous, holistic palliative care for complex cases.
• Coordinate care across GP practices, hubs and PCN.
• Support advance care planning and sensitive conversations.
