Job summary

Marie Curie is recruiting a Senior Healthcare Assistant to join a new South Liverpool service, working across GP practices, community hubs and other locations. You will focus on outreach, signposting and helping people access palliative and end-of-life care earlier, while supporting referrals and navigation of services.

You will work with Clinical Nurse Specialists and primary care colleagues, providing direct care when needed, contributing to holistic assessments and care planning, and

Qualifications

• Knowledge of palliative and end-of-life care.
• Experience caring for people with palliative-end-of-life needs.
• Comfortable having compassionate conversations about serious illness.
• Ability to build trusting relationships with patients, families and health professionals.
• Experience providing person-centred care and support.
• Recognise changes in a person’s condition and elevate concerns.
• Self-motivated and autonomous, planning workload and seeking support when needed.
• Works well in a multidisciplinary team.
• Holds Level 3 Health and Social Care qualification.
• Holds a current valid driving licence and has access to a vehicle.

Responsibilities

• Outreach, building relationships and supporting referrals across GP practices and community locations.
• Help people navigate available care and signpost to appropriate services.
• Follow up identified needs and contribute to joined-up care across PCN.
• Provide direct care when appropriate and contribute to holistic assessment and care planning.
• Support symptom management, advance care planning and compassionate conversations about dying and bereavement.
