Job summary

Marie Curie is recruiting two Clinical Nurse Specialists to join a new service across South Liverpool, working through GP practices, community hubs and PCNs to proactively support palliative and end-of-life care. You will manage a caseload, build relationships across primary and community care, and coordinate care across services, using autonomous clinical judgement.

Qualifications

• NMC registered adult nurse with current registration.
• Nursing degree and Level 6+ qualification.
• Experience in specialist palliative care.
• Ability to assess complex palliative and end-of-life needs.
• Confident in sensitive conversations and advance care planning.
• Autonomous practitioner with caseload management.
• Ability to coordinate care across organisations and refer appropriately.
• Driving licence and access to vehicle.

Responsibilities

• Outreach and referrals across GP practices and community hubs.
• Coordinate care and build connections across services.
• Assess, plan, implement and evaluate specialist palliative-care interventions.
• Provide expert palliative-care advice to colleagues and patients.
• Support advance care planning and patient-informed decision-making.
• Coordinate care across boundaries and advocate for patient-centred care.
