Job summary

Liquid Personnel is actively seeking an experienced Occupational Therapist to support adult social care through timely initial assessments for a client based in Islington.

You will conduct DFG assessments, manage major adaptations, complete Home Safety Equipment assessments, triage referrals, and prepare detailed Rehousing Reports in collaboration with families, social workers and housing teams to deliver tailored solutions.

Qualifications

• HCPC registration as an Occupational Therapist.
• Paediatric social care experience.
• Knowledge of DFG processes, major adaptations, and rehousing protocols.
• Excellent communication and report-writing skills.

Responsibilities

• Conduct DFG assessments and manage Major Adaptations cases.
• Complete Home Safety Equipment assessments to ensure safe and independent living.
• Triaging referrals and prioritising cases effectively.
• Prepare detailed Rehousing Reports to support housing adaptations and relocation needs.
• Work collaboratively with families, social workers, and housing teams to deliver tailored solutions.
