Job summary

Midlands Partnership NHS Foundation Trust is seeking a Band 5 NMC Registered Nurse to join our Community Nursing team based in Perton, South Staffordshire. The role involves delivering palliative and end-of-life care to patients in their own homes with a focus on complex wound and pelvic health care.

You will travel independently across a large geographical area, assess and support patients, and contribute to evidence-based practice within a supportive team environment.

Qualifications

• Degree in nursing or equivalent.
• Mentorship qualification or equivalent.
• Current NMC Registration.

Responsibilities

• Assess new service user referrals and care/support needs.
• Provide care directly to patients on the caseload with a range of nursing needs (including palliative care, complex wound care, pelvic health, medication administration).
• Promote independence and support patient centred care planning.
• Work autonomously, in line with your scope of practice, competence and policies.
• Handover patient information to the wider team, document patient information effectively.
• Escalation of patients' needs as appropriate.

Education
