Job summary
Sussex Community NHS Foundation Trust is recruiting a Band 5 Registered Nurse to join the Adur PCN Team in Lancing, delivering high‑quality care to housebound patients in their own homes and care home settings.
You will work as part of a friendly multidisciplinary team, with a structured induction, ongoing support and development opportunities to build your community nursing skills in wound care, long‑term conditions, palliative and catheter care, safeguarding and hospital discharge support.
Qualifications
- Registered Nurse with current NMC registration.
- Wound management and catheter care experience are desirable.
- Ability to manage long-term conditions and provide safeguarding.
Responsibilities
- Assessment, planning, delivery and evaluation of patient care.
- Wound assessment and management.
- Catheter and continence care.
- Administration of medication, including insulin where competent.
- Supporting patients with long-term conditions and complex health needs.
- Palliative and end-of-life care delivery.
- Safeguarding adults and raising concerns where appropriate.
- Collaborating with GPs, therapists, care homes and the multidisciplinary team.
- Supporting hospital discharge and preventing unnecessary admissions.
- Maintaining accurate clinical records and contributing to care planning.
