Job summary

Qualifications

• Experience supporting people with support needs, families or young people to gain independence.
• Full driving licence and access to a vehicle insured for business use.

• Thorough knowledge of safeguarding vulnerable adults and children.
• Hold or be willing to work towards an NVQ Level 3 Health and Social Care qualification.
• Computer literacy and Microsoft 365 proficiency.
• Good interpersonal skills.

Responsibilities

• Manage a caseload across the Manchester area and help clients gain independent living skills.
• Liaise with Social Workers, Healthcare practitioners, DWP and Police.
• Write detailed reports assessing service users' independent living skills.
• Conduct weekly inspections of service users' properties.
• Support access to education and employment opportunities.
• Promote wellbeing and de-escalate challenging situations as they arise.
• Signpost to mental health, drug rehabilitation services, counselling and education.

Full time role, working 37.5 hours per week across Monday - Friday, with a weekly shift pattern of 9:00am - 5:00pm.

Salary

£26,829.86 per year

About The Role

In Manchester, our services extend to care leavers, family placements, unaccompanied/separated Children, mother and baby placements, as well as adults and NHS referrals. The caseload within our team is diverse and will mainly consist of working with young people and adults from 16+. Our clients present a spectrum of needs, including mental health challenges like psychosis or autism, and some have been in the care system throughout their lives.

We are committed to providing comprehensive support, encompassing independent living skills development, assistance in accessing education or employment opportunities, medication reminders, support with benefits applications, accompanying individuals to appointments, active participation in meetings, and offering crucial emotional support. Our team is dedicated to fostering the well-being and progression of individuals across various age groups and needs within the Manchester community.

Essential Requirements

• Previous experience supporting (not caring) for people with support needs, families or young people to gain independence.
• Travel is essential so must hold a full driving licence and have access to a vehicle insured for business use.
• Must be prepared to travel in the Greater Manchester region.
• Thorough knowledge of safeguarding vulnerable adults and children
• Hold or be willing to work towards an NVQ Level 3 Health and Social Care qualification.
• Computer literate - Good understanding of Microsoft 365
• Good interpersonal skills

Key Duties

• Working across the Manchester area, managing a caseload of Service Users, helping to build independent living skills.
• Liaising with relevant professionals such as Social Workers, Healthcare practitioners, DWP, and the Police.
• Writing detailed reports which assess a service user's independent living skills.
• Completely weekly inspections of Service User's properties.
• Support with accessing and maintaining education/employment.
• Promote physical, mental and emotional wellbeing, de‑escalating challenging situations when they arise.
• Signposting to appropriate services as required, including mental health, drug rehabilitation services, counselling and education.
