Outreach Worker - Rochdale

We are seeking warm, professional Outreach Workers to join our Personalisation service in Rochdale.

Responsibilities include:

1. Providing flexible, personalized support to individuals with support needs such as mental health issues, learning disabilities, or physical health needs.
2. Helping clients develop skills for independent living, enhancing their quality of life and personal development.
3. Engaging with service users at our Rochdale location, building trusting therapeutic relationships, and encouraging positive outlooks and coping skills.
4. Visiting service users in their homes or other settings as part of a structured support approach.
5. Collaborating with other professionals and agencies to deliver coordinated services tailored to individual needs.

Additional details:

• The Rochdale service operates 7 days a week, 365 days a year. Shift work, including evenings and weekends, is required.
