Raising the bar for people-centred outreach services, you’ll enable individuals to learn skills and knowledge to understand and manage their own situations more independently. No matter how complex someone’s mental health needs, you’ll provide support that helps decrease isolation, promote community integration, and maximize opportunities.
The role involves rotational shifts, including evenings, weekends, and bank holidays. However, we are happy to discuss flexible working arrangements to promote work/life balance.
You will support clients across Manchester in the community, encouraging engagement in drop-in activities at community centres, attending medical appointments, and supporting social inclusion activities. You’ll work with clients in their homes, assisting with medication management, daily living activities, and providing emotional support when needed, reporting concerns to the Outreach Manager or Nurse Manager.
Working alongside qualified nurses, you’ll recognize signs of deteriorating mental health and help implement interventions. You’ll promote independence, well-being, and healthy lifestyle choices, and participate in activities such as walks, theatre trips, gym sessions, and social network building.
We are ideally seeking an Outreach Support Worker with experience working with individuals with long-term mental health issues and a solid understanding of their challenges and recovery processes.
You should be a team player with energy, strong communication skills, flexibility, and a positive attitude to meet targets in a dynamic environment. Empathy, understanding, and patience are essential, along with a passion for engaging and helping people.
Having a driving license is highly desirable.
