Raising the bar for people-centred outreach services, you’ll enable individuals to learn skills and knowledge to understand and manage their own situations more independently. No matter how complex someone’s mental health needs, you’ll provide support that helps decrease isolation, promote community integration, and maximize new opportunities.
The role involves rotational shifts including evenings, weekends, and bank holidays. We are open to discussing flexible working arrangements to promote work/life balance.
You will support clients in the community across Manchester by encouraging engagement in drop-in activities at community centres, attending medical appointments, and supporting social inclusion activities. You will work with clients in their homes, assisting with medication management, daily living activities, and providing emotional support when necessary, reporting concerns to the Outreach Manager or Nurse Manager.
Working alongside qualified nurses, you’ll recognize indicators of deteriorating mental health and help implement interventions. Promoting independence, well-being, and healthy lifestyle choices, you’ll also support activities such as walks, theatre trips, gym sessions, and building social networks within the community.
We are ideally seeking an Outreach Support Worker with experience working with individuals with long-term mental health issues and a solid understanding of their challenges and recovery processes.
You should be a great team player with energy, strong communication skills, flexibility, and a positive attitude to achieve targets in a dynamic environment. Empathy, patience, and understanding are essential, along with a passion for helping others.
Having a driving license is highly desirable.
