The Outpatient RN CAP 1 is an entry-level position within the Clinical Advancement Program (CAP). This role involves providing comprehensive patient care, including assessment, planning, intervention, and evaluation, for a designated patient group. Responsibilities include delegating tasks to care staff such as RNs, LVNs/LPNs, and Patient Care Technicians. As part of the End Stage Kidney Disease (ESKD) team, the nurse participates in decision-making, teaching, leadership, and quality improvement activities to enhance patient outcomes and operational efficiency.

Principal Duties and Responsibilities
  • Perform all duties under supervision and guidance, adhering to policies and regulations.
  • Systematically collect and analyze patient data before, during, and after hemodialysis, document findings, and adjust treatment plans accordingly.
  • Assess and educate patients and families about care needs and treatment options.
  • Coordinate and deliver safe hemodialysis treatment with the care team.
  • Administer medications as prescribed, documenting effectiveness and justification.
  • Respond to emergencies as needed.
  • Participate in staff training, orientation, and serve as a resource for the team.
  • Ensure proper laboratory procedures and follow-up on missed collections.
  • Update nursing assessments and care plans collaboratively.
  • Inform patients about transplant and treatment options.
  • Complete CAP requirements for career progression.
  • Perform other duties as assigned.
Physical Demands and Working Conditions
  • Involves heavy lifting, moving patients, and assisting with ambulation, with equipment or coworker assistance.
  • Requires prolonged standing and bending.
  • May need to move equipment weighing up to 200 lbs. and lift solutions of up to 30 lbs.
  • Work environment is a healthcare setting with controlled temperature and moderate noise.
  • Potential exposure to infectious materials.
  • Includes desk, computer work, and interaction with staff and patients.
  • Travel may be required.
Supervision
  • Oversee Patient Care Technicians, LPNs, LVNs, or RNs as a Team Leader or Nurse in Charge upon meeting experience and training criteria.
Education and Licensure
  • Graduate from an accredited Nursing program.
  • Valid state nursing license.
  • CPR/BLS certification.
  • Meet state-specific practice requirements.
Experience and Skills
  • Entry-level RN with less than 2 years of nephrology nursing experience.
  • Chronic/acute hemodialysis experience preferred.
