Job summary

University Hospitals Coventry and Warwickshire NHS Trust, a major teaching Trust in the UK, is seeking an experienced Occupational Therapist to join the Integrated Discharge Team. You will assess functional needs, plan discharge routes, and coordinate support that promotes independence and dignity for patients leaving hospital.

You’ll work with a multidisciplinary team of nurses, physiotherapists, social workers and community partners.

Responsibilities

• Assess, plan and coordinate safe, timely discharges via appropriate discharge pathways.
• Manage a caseload of patients with varying diagnoses and complex needs across disabilities.
• Educate and train the team on the OT role and provide supervision to junior staff.
