Job summary

Wye Valley NHS Trust is seeking a Band 6 senior Occupational Therapist for the Discharge to Assess service, covering City-West and City-East localities. The post focuses on home-based assessments and rehabilitation for people leaving hospital, including equipment and adaptations, with remote working and supervision of junior staff.

You will work with health and social care partners to ensure safe transfers of care, contribute to service development and maintain CPD as part of a proactive

Qualifications

• HCPC registration is required
• OT degree completed
• Post-graduate specialization or substantial post-graduate experience preferred

Responsibilities

• Complete holistic OT assessments in patients' homes after discharge
• Assess ADLs, cognition, mobility, environment and risks
• Develop rehabilitation and reablement plans to maximise independence
• Arrange equipment and home adaptations
• Provide direct rehabilitation and manage a range of conditions
• Collaborate with hospital teams and social care partners to ensure smooth transfers
