Job summary

Doncaster and Bassetlaw Teaching Hospitals NHS Foundation Trust is looking for a Physiotherapist or Occupational Therapist to join the Integrated Discharge Team on a fixed-term basis until March 2028. You will deliver specialist assessments and treatments to drive patient flow and reduce length of stay.

The role supports frailty and medical units and requires autonomous practice with strong collaboration across teams.

Qualifications

• Dual skilled in Physiotherapy and Occupational Therapy competencies.
• Strong communication and interpersonal skills.
• Experience in discharge planning and patient flow improvements.

Responsibilities

• Provide therapy assessments and treatment to support timely discharge.
• Work autonomously within the Integrated Discharge Team.
• Link with community providers to meet patients' therapy and social care needs.
• Support Team Lead with operational management and service development.
