Job summary

Royal Free London NHS Foundation Trust is hiring a Band 4 Patient Pathway Navigator in Oncology for Chase Farm Hospital. The role supports administrative duties and patient navigation from referral to discharge, working within a multidisciplinary team to ensure efficient patient flow.

The successful candidate will handle typing, data entry, patient communications and clinic coordination, with emphasis on empathy and professional conduct in all interactions.

Qualifications

• Typing and data-entry skills.
• Calm, professional appearance and a helpful manner.
• Ability to follow Trust policies and procedures.
• Phone patients to provide instructions, reminders and information.

Responsibilities

• General administrative duties for the department, including faxing, filing and distribution of mail.
• Ensure letters and discharge summaries are sent within agreed timeframes.
• Review actions from transcription services when requested by the team.
• Answer telephone calls and deal with enquiries in a courteous, proactive manner.
• Process requests for clinic changes, cancellations and new appointments in accordance with Trust policy.
• Type up urgent letters when required.
• Keep patient demographics and contact details up to date on Cerner.
• Use computerised tracking systems appropriately when accessing records.
• Phone patients to provide instructions, reminders and information as needed.
• Attend meetings relevant to the role; provide inclusion in MDTs and clinics.
• Provide patient navigation services for satellite and off-site locations.
