Job summary

Royal Free London NHS Foundation Trust is seeking a Band 4 Clinical Pathway Navigator to support Hepatology. The role focuses on coordinating patient care from referral to discharge with a named contact person, in a busy hospital setting.

Fixed term for 12 months (maternity cover), full time 37.5 hours per week, based at Royal Free Hospital, London. Salary £34,186–£37,389 per annum, with NHS benefits and career development opportunities.

Qualifications

• English and maths at GCSE level or equivalent.
• Knowledge of medical terminology.
• Diploma level qualification or equivalent experience.
• Courses/further study attended to demonstrate evidence of personal development.

Responsibilities

• Coordinate the patient care journey with a single named contact.
• Coordinate administrative functions from GP referral to discharge in line with care pathway instructions.
• Assist patients in coordinating multiple pathways to avoid unnecessary appointments across the Trust.
