Job Description

  • Providing practical support and information for Adults and Carers.
  • Formulating and implementing individual support plans by arranging and monitoring the delivery of services/payments to Adults and Carers.
  • Working in conjunction with team colleagues and other professional staff to ensure that service user's needs are met.
  • Monitoring, reviewing, and evaluating services to ensure they are provided effectively and in accordance with needs.
Responsibilities include, but are not limited to:
  • Undertaking assessments of Adults' and Carers' needs in accordance with the Care Act 2014 and departmental procedures.
  • Requesting minor adaptations to the Service User's home, checking their suitability post-installation, and ensuring safety and independence for the Service User and Carer.
  • Seeking advice and support from qualified or registered professionals, e.g., Occupational Therapists, when additional input is needed.
  • Maintaining accurate Adult and Carer records following departmental policies and practices.
  • Following up on complaints in accordance with departmental policies.
  • Engaging with users and carers in a manner consistent with anti-discriminatory practices developed by the department.
  • Contributing ideas and information for future service development.
  • Making appropriate safeguarding alerts in collaboration with professional staff.
Requirements:
  • Experience working within Health and Social Care directly with Adults and Carers.
  • A relevant social and health care qualification is preferable but not essential.
  • Thorough knowledge and understanding of the Care Act 2014, the Mental Capacity Act 2005, and other relevant legislation.
  • Sound working knowledge of Social Services legislation and practice.
  • Ability to develop and maintain positive relationships with individuals and organizations with different and conflicting agendas.
  • Good verbal and written communication skills.
  • Creative and imaginative thinking to anticipate and solve problems within appropriate parameters.
  • Enhanced DBS check dated within the last 12 months; being on the update service may allow for a faster recruitment process.
